Death doesn’t have a gender, but seems to have always depicted itself as “female” in appearance on the Marvel Universes from what I can tell. Doesn’t mean it’s a different character…
currently death is in the Death Stone, that Thanos had. He captured death and created a new stone. As for Death all being the same, I would say each Pantheon had different versions of death.
